PHI means records about you, your treatment and services, including Medical Information and identifiers
Blue Shield says it creates records about you, your medical treatment and the services it provides to you in the normal course of business. It calls the information in those records protected health information (PHI). PHI includes your Medical Information as defined by California law and Personally Identifiable Information (PII) such as your name, address, telephone number and Social Security number, as well as health information such as healthcare diagnosis or claim information.
