Published: June 27, 2025

Overview

Welcome to MyChart Central, an application developed by Epic Systems Corporation (“Epic”) that is designed to streamline access to your MyChart accounts across multiple healthcare organizations. Through MyChart Central, you can link separate MyChart accounts and access them using the same account credentials. Your use of MyChart Central is subject to these Terms of Use (“TOU”), the [Epic Mobile Application Privacy Policy for Patients](https://www.epic.com/privacypolicies/?privacy-policy=mobile-policy-patient) (if you use the MyChart mobile application to access MyChart Central), and the [Epic Website Privacy Policy](https://www.epic.com/privacypolicies/?privacy-policy=epic-privacy-policy) (if you use a web browser to access MyChart Central), each of which may be updated periodically. Carefully read this TOU and the linked privacy policies as they govern your use of MyChart Central and limit Epic’s liability. Each of your healthcare organizations may have additional terms and conditions for your use of their MyChart system.

MyChart Central has an optional feature that can update this information across your linked healthcare organizations. Keeping your information accurate in MyChart Central helps your healthcare organizations maintain correct information about you as well.

Changes to this Agreement; Your Right to Change or Discontinue Using Your MyChart Central Account. Epic may change this TOU at any time by posting a subsequent version on MyChart Central that you will be prompted to accept upon log in. You are responsible for reviewing any updates to the TOU and deleting your MyChart Central account if you do not agree to any such changes. Epic may stop providing MyChart Central or change MyChart Central features and functionality at any time and without prior notice to you. You agree that Epic may terminate your access to MyChart Central and your account without prior notice to you for any reason that Epic deems appropriate. You may stop using MyChart Central at any time. If at any time you want to delete your MyChart Central account and terminate your agreement to this TOU, you may do so using the “Delete My Account” activity within MyChart Central. Deleting your MyChart Central account does not delete any data from your healthcare organizations’ systems. If you delete your MyChart Central account, you are still able to access your health information through your healthcare provider’s MyChart system.

Warranties and Disclaimer. a. Warranties by You. You hereby represent, warrant, and covenant for the benefit of Epic that you have the legal right and authority to enter into this Agreement. You further represent, warrant, and covenant, that: (i) you are not located in a country that is subject to a U.S. Government embargo; (ii) you are not listed on any U.S. Government list of prohibited or restricted parties; and (iii) you will not use MyChart Central for any purposes prohibited by United States law. You may not use or otherwise export or re-export MyChart Central except as authorized by United States law and other laws applicable to You. In particular, but without limitation, MyChart Central may not be exported or re-exported (a) into any U.S. embargoed countries or (b) to anyone on the U.S. Treasury Department’s list of Specially Designated Nationals or the U.S. Department of Commerce Denied Persons List or Entity List. b. DISCLAIMER. TO THE MAXIMUM EXTENT PERMITTED BY LAW, MYCHART CENTRAL (INCLUDING ALL RELATED CONTENT), SERVICES, AND THIRD PARTY MATERIALS ARE PROVIDED TO YOU STRICTLY “AS-IS” AND “AS-AVAILABLE” AND EPIC, ITS THIRD PARTY LICENSORS, AND ITS DISTRIBUTORS DISCLAIM ALL WARRANTIES AND REPRESENTATIONS OF ANY KIND WITH REGARD TO MYCHART CENTRAL, THE SERVICES AND ANY OTHER SUBJECT MATTER OF THIS AGREEMENT, WHETHER EXPRESS, IMPLIED, OR STATUTORY, INCLUDING ANY WARRANTY THAT USE OF MYCHART CENTRAL, SERVICES, OR THIRD PARTY MATERIALS WILL BE UNINTERRUPTED OR ERROR FREE, MERCHANTABILITY, TITLE, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. NO ORAL OR WRITTEN INFORMATION OR ADVICE GIVEN BY EPIC, ITS EMPLOYEES, DISTRIBUTORS, DEALERS, OR AGENTS, WILL CREATE ANY WARRANTIES BY OR ON BEHALF OF EPIC. BECAUSE SOME JURISDICTIONS DO NOT ALLOW THE EXCLUSION OF IMPLIED WARRANTIES, THE ABOVE EXCLUSION OF IMPLIED WARRANTIES MAY NOT APPLY TO YOU.

Limitation of Liability. a. LIMITATION. TO THE MAXIMUM EXTENT PERMITTED BY LAW, IN NO EVENT SHALL EPIC OR ITS AFFILIATES BE LIABLE FOR PERSONAL INJURY, OR ANY INCIDENTAL, SPECIAL, INDIRECT, EXEMPLARY OR CONSEQUENTIAL DAMAGES WHATSOEVER, INCLUDING DAMAGES FOR LOSS OF PROFITS, LOSS OF DATA, BUSINESS INTERRUPTION OR ANY OTHER COMMERCIAL DAMAGES OR LOSSES, ARISING IN ANY WAY IN CONNECTION WITH OR OUT OF THIS AGREEMENT, MYCHART CENTRAL (INCLUDING RELATED APP CONTENT), ANY SERVICES, OR ANY THIRD PARTY MATERIALS, HOWEVER CAUSED, REGARDLESS OF THE THEORY OF LIABILITY (STRICT LIABILITY, NEGLIGENCE, CONTRACT, TORT OR OTHERWISE) AND EVEN IF EPIC HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES. IN JURISDICTIONS WHERE A LIMITATION OF LIABILITY FOR CONSEQUENTIAL OR INCIDENTAL DAMAGES IS NOT PERMITTED, EPIC’S LIABILITY FOR CONSEQUENTIAL OR INCIDENTAL DAMAGES IS LIMITED TO THE MAXIMUM EXTENT PERMITTED BY LAW. b. AGGREGATE LIABILITY. IN NO EVENT SHALL EPIC’S AGGREGATE LIABILITY TO YOU FOR DAMAGES EXCEED THE AMOUNT OF FIFTY UNITED STATES DOLLARS ($50.00). THE FOREGOING LIMITATION WILL APPLY EVEN IF THE ABOVE STATED REMEDY FAILS OF ITS ESSENTIAL PURPOSE. YOU AGREE THAT THE FREE ACCESS TO MYCHART CENTRAL REFLECTS THE ALLOCATION OF RISK SET FORTH IN THIS AGREEMENT AND YOU ACKNOWLEDGE THAT EPIC WOULD NOT ENTER INTO THIS AGREEMENT WITHOUT THESE LIMITATIONS ON ITS LIABILITY.

Support; Contact Information As a MyChart Central account holder, Epic may provide some technical support to you for questions or concerns regarding account access and troubleshooting. You can reach out to Epic for troubleshooting by emailing MyChartCentralSupport@epic.com.

Governing Law and Jurisdiction. This TOU shall be governed by and construed under the laws of the State of Wisconsin, without regard to that State's conflict of laws principles. You agree that the state and federal courts located in Dane County, Wisconsin shall have exclusive jurisdiction over any claim or action under this TOU or related in any way to Epic's operation or your use of MyChart Central, and you agree to submit to the personal jurisdiction of such courts. Notwithstanding the foregoing, you agree that Epic may seek injunctive remedies (or equivalent legal relief) against you in any jurisdiction with regard to any breach by you of the terms of this TOU. The United Nations Convention on Contracts for the International Sale of Goods shall not apply to this TOU or your use of MyChart Central.

Severability. If any provision of the TOU is held, by a court of binding jurisdiction, to be invalid, illegal, or unenforceable, the remainder of the TOU will remain in full force and effect.

Assignment. You may not assign or transfer the TOU nor any rights or obligations that you have under the TOU to anyone without Epic’s written approval. Epic may freely assign the TOU or transfer any of its rights under the TOU.

Arbitration Agreement. a. APPLICABILITY OF ARBITRATION AGREEMENT. Any dispute, claim or controversy of any nature arising out of or relating in any way to MyChart Central, the TOU, or this Arbitration Agreement (as defined below), including the interpretation, validity, scope, or applicability of the TOU or the Arbitration Agreement (hereinafter, collectively, the “Dispute”), which cannot be resolved informally, shall be resolved by binding arbitration on an individual, and not a class or representative, basis under the terms set forth in this Section 14 (the “Arbitration Agreement”). This Arbitration Agreement applies to you and Epic (collectively, the “Parties”) and the Parties’ subsidiaries, affiliates, agents, employees, predecessors in interest, successors in interest, and assigns. THE PARTIES UNDERSTAND THAT THEY WOULD HAVE HAD A RIGHT OR OPPORTUNITY TO LITIGATE DISPUTES THROUGH A COURT, TO HAVE A JUDGE OR JURY DECIDE THEIR CASE, AND MAY HAVE HAD A RIGHT OR OPPORTUNITY TO PARTICIPATE IN A CLASS ACTION OR OTHER PROCEEDING INVOLVING MULTIPLE CLAIMANTS, BUT THEY INSTEAD CHOOSE AND AGREE TO HAVE ALL DISPUTES AS SET FORTH HEREIN DECIDED THROUGH INDIVIDUAL ARBITRATION. b. DISPUTES EXCLUDED FROM ARBITRATION. In the event of any actual, alleged or threatened violation of confidentiality or violation of Epic’s intellectual property or other proprietary rights, Epic may immediately resort to court proceedings in a court of competent jurisdiction as set forth above in order to seek immediate injunctive relief without posting of a bond, proof of damages or other similar requirement. The institution of any action for injunctive relief shall not constitute a waiver of the right or obligation of any party to submit any claim seeking relief other than injunctive relief to arbitration. c. CONFIDENTIAL PROCEEDINGS. The Parties expressly agree that any and all actions taken under the Arbitration Agreement and related provisions, including but not limited to all filings, orders, judgments, and awards made in any arbitration proceeding, are confidential and may not be disclosed to any third party, except as may be required by law. d. RULES AND FEES. The arbitration proceeding will be administered by the American Arbitration Association (“AAA”) before a single arbitrator under the AAA's Commercial Arbitration Rules. Epic will pay all filing, administration, and arbitrator fees associated with the arbitration. The decision of the arbitrator will be final and binding. Any final award or judgment may be filed and enforced in any court of competent jurisdiction. Further, unless both you and Epic agree otherwise, the arbitrator may not consolidate one or more person’s claims with your claims, and may not otherwise preside over any form of a representative or class proceeding. If there is a final judicial determination that applicable law precludes enforcement of this Paragraph's limitations as to a particular claim for relief, then that claim (and only that claim) must be severed from the arbitration and may be brought in court. e. PLACE AND GOVERNING LAW. The place of arbitration shall be in Dane County, Wisconsin, unless otherwise agreed to in writing by all parties to the arbitration. The Parties acknowledge that this Arbitration Agreement evidences a transaction involving interstate commerce, and the Federal Arbitration Act, 9 U.S.C. § 1, et seq., as may be amended from time to time, shall govern the interpretation, enforcement and proceedings pursuant to this Arbitration Agreement. To the extent state law is applicable, the laws of the State of Wisconsin shall apply without regard to conflict of laws provisions. f. TIME LIMITATION ON CLAIMS. The Parties agree that they must initiate arbitration within one (1) year after any Dispute arose; otherwise, the Dispute is permanently barred.

Interpretation; Entire Agreement. Headings in these TOU will not affect the interpretation of these TOU. In these TOU, the words “include” and “exclude” and their variants are not words of limitation, and examples are for illustration and not limitation. This TOU, the [Epic Website Privacy Policy](https://www.epic.com/privacypolicies/?privacy-policy=epic-privacy-policy), the [Epic Mobile Application Privacy Policy for Patients](https://www.epic.com/privacypolicies/?privacy-policy=mobile-policy-patient), and the MyChart Application End User License Agreement constitute the entire agreement between you and Epic regarding MyChart Central and supersede all prior agreements, representations or understandings respecting MyChart Central. Nobody, including any representative of Epic, is authorized to make any changes or modifications to the terms of this agreement other than changes that are instituted through the authorized posting of subsequent versions of this agreement as discussed above. You are not entitled to act or rely on any representations to the contrary. A failure by Epic to enforce any provision of this agreement, or a waiver by Epic of any breach of this agreement, shall not act as a bar to subsequent enforcement of such provision or be deemed a waiver of any subsequent breach.
